A new parameter for the determination of normal right ventricular function in patients with acute pulmonary embolism

چکیده

Background/Aim: The performance of the right ventricular myocardium is crucial in various pathological states and dysfunction has a prognostic value pulmonary embolism. We sought to bring out which parameters were helpful predicting normal function patients with acute Methods: Consecutive 100 embolism patients, who hospitalized confirmed by computed tomography angiography, enrolled this cohort study. All patients’ demographics, symptoms on admission, risk factors, electrocardiography laboratory findings, hemodynamic assessed. Echocardiography was performed first 24 hours. study group divided into two groups based their basic characteristics: Patients failure. Results: average age 63 (16) years, 48 (48%) them being male. Twenty three (23%) had RV functions. According multiple logistic regression analysis, (P=0.041, OR: 1.174, 95% CI: 1.007 1.368), oxygen saturation (P=0.026, 1.372, 1.039 1.812) heart rate (P=0.049, 1.160, 1.001 1.346) independent predictors function. setting all (Age, Heart rate, Oxygen saturation) positive considered AHO index=1, predictive 100% sensitivity 44%, negative 85.6% specificity (AUC: 0.717, 0.619 0.803) for Conclusion: In younger than 53 years ≤118 bpm an >90% (AHO index=1), functions normal. Accordingly, without need angiography or echocardiography, clinician may predict available demographic noninvasive parameters.

Determination of the Effects of Digoxin on the Right Ventricular Function in Patients Undergoing Pneumonectomy

Introduction: Pneumonectomy is the standard treatment of lung cancer, even though patients should undergo several evaluations before surgery; deterioration of cardiopulmonary function after pulmonary resection is inevitable. We have evaluated the effects of digoxin on the improvement of right ventricular function and prevention of probable complications after lung resection surgery. Materials a...

Association between right ventricular function and perfusion abnormalities in hemodynamically stable patients with acute pulmonary embolism.

BACKGROUND/OBJECTIVES Patients presenting with acute pulmonary embolism associated with hemodynamic compromise exhibit right ventricular enlargement and dysfunction on transthoracic echocardiogram. However, the degree of echocardiographic abnormalities among hemodynamically stable patients without preexisting cardiopulmonary disease during the acute stage of pulmonary embolism, and following tr...

The value of isovolumic acceleration for the assessment of right ventricular function in acute pulmonary embolism.

AIM The aim of this study was to assess the value of tricuspid annulus myocardial isovolumic acceleration (IVA) in the assessment of right ventricular function in patients with acute pulmonary embolism (PE). METHODS Fifteen patients (mean age 60.6±11.3 years) with acute PE were enrolled and a control group was formed of 15 patients with a similar mean age (60.3±11.5). Patients who were diagno...

Saddle pulmonary embolism with normal right ventricular function: a treatment enigma

Management of saddle pulmonary embolism (PE) in haemodynamically stable patients with normal right ventricular (RV) function is unclear. Thrombolytic agents are indicated in haemodynamically unstable patients with PE. RV dysfunction has also been used as an indication for surgical embolectomy in normotensive patients with PE. Embolized thrombus burden, although intuitively related both to haemo...

Acute improvement in right ventricular function after treatment of presumed massive pulmonary embolism with thrombolytics

Acute massive pulmonary embolism results in a rise in pulmonary vascular resistance and acute right ventricular dilation and systolic dysfunction. In unstable patients who cannot safely undergo computed tomography, echocardiography can often provide direct and indirect evidence of pulmonary embolism. We present a case in which clinical and echocardiographic evidence provided sufficient evidence...
